| 中文摘要 | 目的探讨多层螺旋CT冠状动脉造影成像的临床应用价值。方法104例冠状动脉进行多层螺旋CT血管造影,用平扫图像行钙化积分测定,利用增强扫描重建像行多平面重建(MPR)、最大密度投影重建(MIP)及容积再现技术重建(VRT),其中11例患者有常规冠状动脉造影作对照,总结CT冠状动脉造影的评价方法和显示情况,分析影响冠状动脉成像的原因。结果16层螺旋CT平扫可测定钙化积分,增强扫描MPR、MIP及VRT重建像能多角度各向同性地显示冠状动脉的所有1级、绝大部分2、3级以及部分4级分支。MPR及MIP图像对冠状动脉分支的显示优于VRT图像。VRT重建像图像质量的优良率:优75.0%(78/104),良14.4%(15/104),差10.6%(11/104),优良率为89.4%。图像质量影响因素有主动脉根部造影剂浓度、心率、有无心律不齐和患者屏气等。发现冠状动脉管壁欠光滑53例,管壁钙化斑块50例,非钙化斑块24例,管腔狭窄36例,管腔闭塞1例,搭桥血管2例,血管内支架6例。结论16层螺旋CT冠状动脉血管造影是一种无创、简便、优良的冠状动脉成像方法,可作为冠状动脉粥样硬化疾患的筛选手段及冠状动脉术后复查的首选方法。; Objective To evaluate the clinical application of multislice spiral CT coronary angiography.Methods 104 cases of coronary artery were examined by multislice spiral CT.Calcium scoring with plain scan images and MPR,MIP and VRT reconstruction with enhanced scan images were made in all cases,among which 11 cases underwent conventional coronary angiography.The evaluation methods and demonstra-tion were summarized.The factors influencing the image quality were analyzed.Results Coronary calcium score could be got by plain scanning images.All of the primary,most of the secondary and tertiary,and part of the fourth level branches of coronary artery could be shown isotropically with MPR,MIP and VRT reconstruction images of en-hanced scanning images.MPR and MIP images were better than VRT images in demon-strating small branches.The ratio of VRT fine image quality:excellent 75%(78/104),good 14.4%(15/104),bad 10.6%(11/104),and the ratio of fine images is 89.4%.The image quality was correlated to the contrast concentration in the root of aorta,heart rate,existence or absence of arrythmia,and the patient's breathholding ability.53 cases were shown as insufficient smooth,and 50 cases with calcified plaque,24 cases with noncalcified plaque,36 cases with stenosis,1 cases with obliteration,2 cases with pass-by artery,6 cases with stent.Conclusion 16-slice spiral CT coronary angiography is a noninvasive,simple and excellent method.It is able to be used as a preferred method for coronary atherosclerosis and postoperative check. |
